This policy addresses information collected through the website. The way we handle Protected Health Information (PHI) as a healthcare provider is described separately in our Notice of Privacy Practices, which governs the clinical care relationship.

If there is any conflict regarding PHI, the Notice of Privacy Practices controls.

We may update this Privacy Policy from time to time. When we do, we'll revise the "Last updated" date above and post the current version on this page.
